Authority and fitness
I confirm that I am the player’s parent or legal guardian and may authorize participation. I understand that soccer is physically demanding and will not allow participation when the player is ill, injured, or medically unable to play.
Risks
I understand that soccer and attendance at an athletic facility involve risks including falls, collisions, contact with players or equipment, weather exposure, and serious injury. I knowingly accept the ordinary and inherent risks of participation.
Emergency care
If I cannot be reached, I authorize Championship Seven personnel to seek reasonable emergency assistance for the player. I remain responsible for medical costs not covered by applicable insurance.
Release
To the fullest extent permitted by Texas law, I release Championship Seven, its operators, officials, contractors, and participating facility from claims arising from ordinary negligence connected with participation, except claims that cannot lawfully be waived. This does not release gross negligence or intentional misconduct.
Rules
I will ensure the player and accompanying spectators follow league and facility safety and conduct rules. Championship Seven may suspend participation when needed to protect players, officials, or the event.
